Going Home
NorthEast Critical Care physicians care for your health needs while in the Intensive Care Unit (ICU)
at the hospital and release all records to your Primary Care physician. Once you are home, patients
are encouraged to follow up with their primary care physician for any healthcare needs that may
arise.
When you are well enough to leave the Intensive Care Unit, you may be sent to a regular care floor
or a step down unit. The physicians of NorthEast
Inpatient Services will oversee your medical care while on a regular care floor.
Being Discharged
When a hospitalized patient is released from the hospital, either to go home or to another care facility,
our Case Management
Department is there every step of the way. Registered Nurses and Clinical Social Workers can assist
patients and their families with any needs they may have, from insurance to home health care and everywhere

Area Services
If special care is needed upon being released from the hospital, our home healthcare department,
Healthy@Home, can provide physical,
occupational, and speech therapy services either in your home or in several outpatient facilities in our area.
In addition, terminally ill patients and their families may need the extra care and support that
Hospice and Palliative Care of
Cabarrus County can provide.
